This is a brilliant opportunity for a Social Care Assistant looking for a career in an NHS and social care role. Working in a multi-disciplinary team, you would be providing services to people in the London Borough of Redbridge on both it and the NELFT NHS Foundation Trust's behalf. Only motivated and skilled individuals need apply and your role in the Central Assessment Review Team (CART) would be to indentify and achieve cost savings. Alongside district nurse, social workers, rehabilitation therapists, occupational herapists, mental health and learning disability colleagues, the role requires carrying out Care Act assessments of care packages.

Reporting directly to the Senior Social Worker and Head of Service, you'll need to be an enthusiastic individual to help the council to deliver: A strength-based approach using People Matter model through the 3-conversation model. Person-centred holistic support planning to support people to maintain their independence by promoting health and wellbeing. A joint assessment approach, which covers both health and social care needs, and a single point of contact for people and their carers. Assessments for minor pieces of equipment and adaptations which will support the person to remain independent. 

After the initial 2 week full time training period, the role is hybrid 2 days a week in the office and 3 from home.
